legendary pickaxe

Hi, I was afk and got timed out. When I reconnected this morning I was surprised to see my Slot #1 on my hotbar was empty. This happend 3 weeks ago and happening again. My legendary pickaxe had vanished. It was from a legendary chest and had mending on it .Is there any staff member who knows...
